I’m a professional managed care contract negotiator for a major health system in the US who also teaches health administration classes at a local university. Let me try to be comprehensive:

  1. There’s a difference between what’s price and cost. Hospital prices are what hospitals charge for a service. Like any other business, charges include the direct costs (supplies, clinician time, etc.) and indirect costs (power bill, departmental overhead, etc.). Included in the indirect cost is always an amount to cover uncompensated care. This is cost incurred by people who can’t or don’t pay their bills, and it’s re-allocated into future pricing, creating inflated charges, which are reported as the “price” for healthcare services in the media and quoted by politicians. edit In addition, government programs with set payment systems (i.e Medicare, Medicaid) often cover some, but not all of the direct cost of care. This too is passed on in the pricing.

A lot of people here think this is about knowing something (like what an automatic thought is or how to do a breathing exercise) or having insight (recognizing your own motives and biases). Those people aren’t exactly wrong, but that isn’t the answer to your question.

Really, there are a certain group of people for whom self-help books work quite well, and those are the people for whom the only thing they don’t have is information. But most people don’t do well with self-help books, because they’re lacking something more than information.

Yes. In fact, in the 1939-40 war against the original allies of France, Poland and Great Britain, Germany did win. 

Today, we see these victories as inevitabilities. To the world in 1940, they were nothing short of unimaginable. These victories are not just tactical:

-Poland ceases to exist; 
-France’s Third Republic is replaced by the Nazi collaborationist Vichy government; 
-Great Britain’s forces are pushed off the continent, barely clinging to survival. 

By the summer of 1940, Hitler’s stage-by-stage plan (‘Stufenplan’) as dreamed in the 1920’s is coming to fruition:]\

– Austria, Bohemia and Poland are now part of Greater Germany.
 – France and the Treaty of Versailles are neutralized.
 – The British, Hitler believes, are finished and will soon negotiate.

The Joker and Batman are each trying to prove a point to society – and really to us, the readers.

The Joker wants Batman to kill him because he perfectly embodies chaos and anarchy and wants to prove a point to everyone that people are basically more chaotic than orderly.

This is why he is so scary: we are worried he may be right.

If the Joker is right, then civilization is a ruse and we are all truly monsters inside. If the Joker can prove that Batman – the most orderly and logical and self-controlled of all of us – is a monster inside, then we are all monsters inside, and that is terrifying.
